To register with the club, you need to complete the following form as well as the Declaration (link at the bottom of the page). Once you have completed these forms, you are committing to register for the club, and pay the registration fees.

Declaration

Declaration

I/we give permission for my child (named above) to attend training and to represent Wakefield Athletic JFC during 2025/26 season (in friendly matches, competitive matches and tournaments).

Child protection

I/we give permission for the club to take photographs and to publish match reports in which players are identified by name and, that such photographs/videos , including the one uploaded in this form, can be used for the promotion of the club, including, but not limited to, publication on social media, in match reports, Player of the match awards and newsletters. I further agree to my child’s name to be included in such media coverage where applicable.

Medical Treatment

I/we give permission for club officials to provide first aid to the player in the event of illness or injury. I/we understand that whilst the club’s coaches, officials and helpers will take all reasonable care and precautions to ensure the health and safety of players, they cannot be held responsible for any loss, damage or injury suffered by the player whilst travelling to and from fixtures or whilst participating in matches or training sessions.

Code of Conduct

Respect

Termination

I/we understand Wakefield Athletic JFC reserves the right to cancel membership without refund, in the event that conduct, on or off the pitch, is deemed by The Club to be unacceptable.

Leaving The Club

I/we understand that if I/we choose to leave the club at any point during the season, I/we will not be entitled to a refund of registration fees.
